South Park 2010 Football

Dear South Park Parent,

Our first major event of 2010 will be our annual football camp held in June at Carmel Middle School. The camp allows veteran players to hone their skills in preparation for the new year and at the same time allows newcomers to measure their interest level prior to the fall season.
This 4 day non-contact youth football camp is designed for energetic players’ ages 7 through 13 years old who want to learn about football and have fun. Participants will be grouped according to age and ability levels and will receive a camp t-shirt and pizza party. The camp will run from June 14th (Monday) through June 17th (Thursday) from 6:00 PM until 8:45 PM each evening. 2010 Football Camp Registration Form.
Participants who played football for South Park in 2009 or who attend our 2010 football camp will be given a priority at the June 19th sign-ups. The following sign up dates are on a first come first serve basis. We highly encourage parents to review the required documents and sign up at the earliest opportunity as we regularly fill our slots each year and are reluctantly forced to start waiting list(s).
Parents are encouraged to review the South Park Overview to better understand the football program.

Saturday June 19th from 9:00 AM until 1:00 PM – Priority sign-ups for previous players
Saturday July 10th from 9:00 AM until 1:00 PM – Priority sign-ups for previous players
Tuesday July 13th from 6:00 PM until 8:00 PM – Open sign-ups
Saturday July 17th from 9:00 AM until 1:00 PM – Open sign-ups
Saturday July 24th from 9:00 AM until 1:00 PM – Open sign-ups
We will be selling South Park t-shirts and shorts for kids and adults as well as practice jerseys, practice pants and mouth pieces during sign-ups. These items promote our organization and allow the kids to feel proud of this endeavor.
We plan to have a registered nurse give physicals at each of our sign-ups. These sports physicals are a yearly requirement to participate in the Pop Warner football program. The registered nurse charges a $20.00 cash fee for each physical. Each participant must have a physical after 1 January 2010 and prior to 1 August 2010 to participate in Pop Warner Football. There are NO exceptions.
We can only accept complete registrations. Until a registration packet is 100% complete, a player cannot reserve a roster spot and is not considered enrolled in the SPYA football program. We cannot accept or process fall football registration packets through the mail.
The five (5) required documents for the fall program are outlined below.
Participation Check List:
Birth Certificate: a Xeroxed copy which we will keep. Do not bring originals.  You must submit a copy each year even if your son played for South Park last year.
Report Card: a Xeroxed copy which we will keep. The copy of the report card must clearly show every grade from every grading period for the entire school year. Do not bring originals.
2010 Participant Contract Form: (2 page form) general information and parental consent. After the parent signs this form; it cannot be duplicated or faxed. We must have this original Pop Warner Form.  Click Here to download the form.
Pop Warner 2010 Physical Fitness & Medical History Form: (2 page Form) detailed medical information and the doctor’s release. No other health form of any kind is acceptable for Pop Warner Football. After the doctor signs this form, it cannot be duplicated or faxed. We must have this original Pop Warner Form signed by a doctor and it must be completed and signed in 2010.  Click Here to download the form.
